We are currently accepting new patients!

In order to apply you MUST already posses a valid Washington State Medical Marijuana Authorization. 

Instruction for new patients: 

1. Fax a copy of your Washington State Medical Marijuana Authorization and Washington State photo ID to 253-638-3639. You will be contacted within 24 hours to set an appointment.

2. You will need to complete a membership application and caregiver provider/patient form (both can be viewed under forms) at first appointment.
