In order to apply you MUST already posses a valid Washington State Medical Cannabis Authorization.
Instruction for new patients:
Get Verified. You can get verified by clicking here and submitting your information online, or, by calling 425.251.6060 and giving the receptionist the following information:
- First and Last Name
- Date of Birth
- Clinic and Doctors Name
- Expiration date of medical recommendation
- Phone number to reach you back once you have been verified.
Once you have been verified, you can set an appointment. You will need to bring your WA State ID and the original copy of your medical cannabis recommendation at your first appointment.
