We accept all patients as long as you have a valid medical marijuana recommendation according to RCW 69.51 and a valid state ID.
- To apply for membership in our patient network you must have a valid Washington State Medical Marijuana Recommendation. If you do not, but believe you are a potential patient, you can get more information here on how to obtain your permit.
- You must have a copy of your MMJ recommendation and WA photo ID.
- You must fill out our Caregiver Authorization Form and our membership application.
- Once you have these documents, you may fax or email your MMJ recommendation, ID, HIPAA form, and provider form to us at xxx.xxx.xxxx or Thcf420@gmail.com. If you are unable to fax or email these documents, you can get pre-verified by clicking here, then bring your documents in with you on your first visit.
